https://www.facebook.com/groups/1950335895703525
A nurturing public support network for people, over 45, who, are primary carers for loved ones facing health challenges and end of life, or who are personally facing the same challenges.
Swan Song Community was inspired Kate's life experiences, the few years caring for her Mum and her work as a Doula. Along these journeys, Kate's learned the profound impact of discussing, planning, and documenting one's wishes to ensure holistic care during life's transitions.

Kate is dedicated to supporting individuals, especially women over 45, who are primary carers or facing health challenges themselves. Her mission is to provide a space for reflection, exploring personal experiences, any regrets, and understanding adaptability to change”.
This community was also born out of Kate's passion to connect like-minded individuals who are keen to share their perspectives on life, consider and embrace holistic care before and when facing their mortality.

Acknowledgement of Country
In the spirit of reconciliation, Kate Leotta from Swan Song Journey acknowledges First Nations People, the Traditional Custodians of the lands in South East Qld (and beyond) on which I live and work. I honour their deep, enduring connections to land, sea, and community.
From my heart, I pay my respects to Elders past, present, and emerging, and to all the families who hold the stories, wisdom, and strength of culture—Grandparents, Mums and Dads, Aunties, Uncles, Cousins, Nieces, Nephews, and Kin. I recognise and stand in solidarity with Brotherboys, Sistergirls, and all members of the LGBTQIA+ community and the FPDN within First Nations cultures.
Sovereignty was never ceded—this always was, and always will be, First Nations People's land.
